Clinical Pharmacist

North Island-based Hospital has an opening for an experienced NZ registered Clinical Pharmacist to provide pharmaceutical care to identified patients in order to achieve the safe, effective and cost effective use of pharmaceuticals within the limits of resources.

Medicines Management encompasses the entire way that medicines
are selected, procured, delivered, prescribed, administered and reviewed to optimize the contribution that medicines make in providing informed and desired outcomes of patient care.

Part of your task is to identify pharmaceutical needs, validate medication histories, review medication, monitor treatment and most importantly ensure the patient receives information, support and advice to manage their medicine.
